All Care Health Center strives to provide quality affordable healthcare to all members of the community.

All patients are eligible to apply for All Care’s sliding fee scale. Eligibility is based solely on family size and income. Patients that are approved for the sliding fee scale are eligible for discounted services.

Sliding Fee Scale

The sliding fee scale is a tool All Care uses to determine how much patients are responsible for after receiving medical/dental care. The sliding fee scale is based on family size and income. In order to qualify for our sliding fee scale, the patient will need to complete an application at the time of the visit and provide proof of income within 30 days. No patient will be refused services based on their ability to pay.

The sliding fee scale is based solely on family size and income. For more information please view the Federal Poverty Guidelines.

Nominal Fee

The nominal fee is due at the time of your medical or dental visit. The nominal fee will be for the office visit only. Additional charges may apply for lab, x-ray, and dental surgery/rehabilitation services.

The minimum medical fee is $35 and the minimum dental fee is $55.

All Care Health Center accepts cash, check, Visa, MasterCard or Discover, most forms of insurance, Medicare, and Medicaid. Please bring a copy of your insurance information with you to your appointment.

Good Faith Estimate

Beginning January 1, 2022, patients without insurance have the right to receive a “Good Faith Estimate” to explain scheduled medical care costs. Under the law, health care providers need to give patients, who do not have insurance or who are not using insurance, an estimate of the bill for medical items and services.

Uninsured patients have the right to receive a Good Faith Estimate for the total expected visit cost, including related costs of medical tests, prescription drugs, and equipment fees.

Your Good Faith Estimate should be available, in writing, at least one business day before your medical service or item. You can also ask your health care provider for a Good Faith Estimate before you schedule an item or service.

The Good Faith Estimate does not include any unknown or unexpected cost that may arise during your medical appointment. You could be charged more if complications or special circumstances occur. If this happens, federal law allows you to dispute this bill if the difference between the Good Faith Estimate and the actual charges is greater than $400.00.

This Good Faith Estimate is not a contract and does not require you to obtain the items or services from All Care Health Center. Make sure to save a copy or picture of your Good Faith Estimate.

All Care Health Center strongly agrees that patients should not be subject to “surprise billing,” and that cost should not pose a barrier to care for any patient, including those who are uninsured. This commitment is at the heart of our Federally Qualified Health Center model, which has been in existence for over 50 years. All Care Health Center is working on Good Faith Estimate processes to limit any potential patient confusion and impact to our model of care and minimize additional administrative demand.
