Mission Moments: Carmen’s Story

When Carmen reached out to our Patient Navigator for help getting an INR meter for home use, it opened the door to a life-changing solution for Carmen.
An INR meter is a small medical device that helps patients check how quickly their blood clots. People who take blood thinner medications often need regular INR testing to make sure their medication is working safely. Usually, this means frequent trips to a clinic or lab for blood draws.

At first, our Patient Navigator had never worked with a home INR monitoring system before. But after researching options, they discovered a local company that could provide the equipment, training, and support right in the patient’s home.
The process took teamwork and persistence. After a few unsuccessful attempts to fax paperwork to the durable medical equipment (DME) company, a sales representative personally visited the clinic to collect the orders in person. Once the paperwork was submitted and insurance approval was received, Carmen received her home monitoring system and training within about a month.
Just one week later, Carmen was still excited about her new equipment. Not only does the home INR meter help her stay on top of her health, but it also saves her trips to medical appointments and gives her more time with the people who matter most — her family.
