All Care Awarded Senior Impact Catalytic Grant from Immanuel Vision Foundation

All Care Health Center is honored to announce it has been awarded a Senior Impact Catalytic Grant from the Immanuel Vision Foundation to support the organization’s Senior Health Equity Transformation initiative, a program designed to strengthen care, safety, and access for older adults in our community.

The Immanuel Vision Foundation has spent nearly a decade investing in nonprofit organizations across Nebraska and Iowa whose missions align with its commitment to “meet the physical, emotional, and spiritual needs of seniors, respond to needs in community health, and support the ministry of the Church.” Through its special category of Senior Impact Catalytic Grants, the Foundation provides larger, focused investments in programs addressing significant issues impacting seniors.
All Care’s initiative was developed in response to the growing and increasingly complex healthcare needs of older adults. As the number of seniors served by All Care continues to rise, the organization has recognized the need for enhanced support around medication safety, chronic disease management, preventive care, and healthcare navigation.
The Senior Health Equity Transformation initiative will strengthen senior-focused services through expanded care coordination, clinical pharmacy support, Medicare navigation assistance, preventive wellness visits, and strategies that help reduce barriers to care. The program also emphasizes helping seniors remain healthy, independent, and safely connected to care and community resources.

The grant reflects a shared commitment between All Care Health Center and the Immanuel Vision Foundation to improving the well-being of older adults and strengthening systems of care that help seniors thrive.
