All Care Health Center began in 1997 as Council Bluffs Community Health Center.

Council Bluffs Community Health Center
The Council Bluffs Community Health Center was founded to provide accessible, comprehensive, and culturally competent primary care for the entire community—especially those facing barriers to care.
Our first location was at Place 35 in western Council Bluffs. As the need for services grew, the clinic moved to the Omni Center in 1998 and became a Federally Qualified Health Center (FQHC).
By 2009, planning began for a larger facility to better serve the community. At that time, services included medical, dental, and mental health care provided by a small but dedicated team of providers.
All Care Health Center
In 2012, All Care Health Center moved into its current location at 902 South 6th Street. This expansion added a dental clinic and pharmacy, along with transportation services to help reduce barriers to care.
Florence M. Lakin Health Center
In 2019, All Care opened a second location on the Charles E. Lakin Human Services Campus. The clinic began with medical and dental services, as well as help with insurance enrollment.
Mobile Medical Care
In 2022, All Care launched a mobile clinic in partnership with the Council Bluffs Community School District, starting at Thomas Jefferson High School.
By 2024, services expanded to multiple schools and community locations. The mobile clinic provides primary care throughout the school year and at community sites during the summer.
Substance Use Disorder (SUD) Services
In 2025, All Care began offering Substance Use Disorder (SUD) services to support individuals and families affected by addiction. This expansion reflects our commitment to whole-person care and increasing access to behavioral health services.
