The Sliding Fee Scale helps determine how much you will pay for medical and dental services at All Care.

If you qualify, your cost will be reduced based on our approved fee scale. This means you will pay a discounted rate for covered services.

Your application must be completed and approved before the discount can be applied.

• All patients can apply (even if you have insurance)

• Discounts are based only on:

  • Family size

  • Household income

• No one will be refused services because of inability to pay.

• We use the current Federal Poverty Guidelines to determine eligibility.
