All Care Health Center Nationally Recognized for Quality
All Care Health Center has been nationally recognized for delivering high-quality healthcare after earning eight 2026 Community Health Quality Recognition (CHQR) badges from the U.S. Health Resources and Services Administration (HRSA). The awards recognize excellence in clinical quality, patient outcomes, access to care, and the effective use of health information technology.
Among the recognitions, All Care earned National Quality Leader awards in Diabetes Health, Behavioral Health, and Heart Health, reflecting exceptional performance in three of the nation’s most significant health priorities.

Community Health Quality Recognition badges are awarded annually by HRSA to health centers that demonstrate measurable achievement in areas that matter most to patients. Together, these recognitions reflect excellence in preventive care, chronic disease management, behavioral health, patient access, and the delivery of high-quality, cost-effective healthcare.
The awards reflect All Care’s commitment to providing integrated, patient-centered care. Medical, dental, behavioral health, pharmacy, and enabling services—including care coordination, transportation, interpretation, insurance enrollment assistance, and other patient support services—work together to reduce barriers that often prevent individuals and families from receiving timely healthcare. By coordinating services across disciplines, All Care helps patients stay connected to care, better manage chronic conditions, and achieve healthier outcomes.
